We are pleased to offer the pre-operative dietician package to you at NO UP FRONT COST. This arrangement is for primary bariatric surgery patients, Lap Band revision patients, and bariatric surgery patients after Gastric Bypass and Sleeve with marginal ulcers, GERD, and hiatal hernia.

Your pre-surgery visits with a dietician will include the following:
- One initial one hour individual assessment office visit
- One 3-1/2 hour group bariatric education session involving training and education in preoperative nutrition and habits
- One three hour group preoperative education session involving training and education in diet, exercise, and postoperative expectations.
Prior to your first appointment with the dietician, please contact your insurance company to find out if the individual office visit and two group sessions are a covered benefit under your plan.
The codes the insurance company may ask for are:
1 – 97802 for initial assessment in office visit and;
2 – 97804 for the group counseling sessions.
St. Agnes Healthcare will bill your insurance company for each visit. Any copays, co-insurance, or non-covered services will then be billed to you by St. Agnes Healthcare. If the dietician services are not covered by your insurance plan, you will be responsible for a charge which is mandated by our contractual agreements. We will extend any allowable possible discounts to self-pay patients and insured patients without coverage of bariatric dietician services and provide you with proof of necessary medical service should you have FLEX or HSA.
